Webinar Synopsis: Closing the loop between the consumer and his or her circle of care is paramount to improving the patient (or consumer) experience, improving the health of populations, and reducing the cost of health care. The webinar will feature two case studies of CARETILES in action. Learn how the Mississippi Adolescent Center, a state-operated program for adolescents with an I/DD in Brookhaven, MS and Elwyn, a large multi-state provider organization in Elwyn, PA are using talkingtiles to empower consumers to take part in their own care plans and build independence through learning and therapy skills. By taking what they have learned in the classroom, talkingtiles is helping individuals enhance their quality of life as they transition back into the community.

This 60-minute executive briefing will take place on May 1st, 2014 at 2:00 PM EST and will:

Donna Horton

 

  Donna Horton, Director of Client Services, Mississippi Adolescent Center

 
Donna Horton is currently the Autism Coordinator and Director of Client Services at the Mississippi Adolescent Center in Brookhaven, MS. She earned her Bachelor’s of Science degree from Mississippi State University in 1982; her Master’s of Business Administration from Belhaven University in 2000 and her Master’s of Health Services Administration from Mississippi College in 2006. During her 14 years at MAC, she has worked as an accountant/auditor, Qualified Intellectual Developmental Disability Professional and Interdisciplinary Team Coordinator. She graduated from the Certified Public Manager program in 2009. Donna is licensed as a Nursing Home Administrator and also serves in this capacity at the Mississippi Adolescent Center.

Christina Raftovich

  Christine Raftovich, Manager, Data Governance, Elwyn

 
Ms. Christine Raftovich is currently the Manager, Data Governance for Elwyn, where her professional career exceeds 30 years. Within her Data Governance role, Christine functions as a clinical liaison for the organization and is a dedicated resource to the enterprise implementation of the CoCentrix Coordinated Care Platform. Her previous position was Program Manager for Elwyn’s Community Residential Services (CRS) where she managed community based residential settings for individuals with intellectual disabilities and behavioral disorders. During her fifteen years with CRS, Christine provided both the operational and clinical supervision to her residences and was responsible for maintaining regulatory compliance. Christine is a graduate of Neumann University with a Bachelor of Science degree in Liberal Arts with a concentration in Information Science and a Master of Science degree in Organizational and Strategic Leadership.

Prior to joining CoCENTRIX, Bill held senior leadership positions with Allscripts where his sales teams focused on enterprise Care Coordination, Post-acute, ambulatory EHR and Population Health solutions. Prior to Allscripts, he was a key contributor and leader with a start-up company in the Care Management and Discharge Planning space – Extended Care Information Network (ECIN). Prior to that, he held sales and sales leadership positions with McKesson/HBOC and Baxter Healthcare.

Bill holds an undergraduate degree in Business (BBA) in Management Information Systems from the University of Iowa and an MBA in marketing from DePaul University in Chicago where he was a member of the Delta Mu Delta National Honor Society for Business Administration.
